The apocalypse challenge was added to the game before Insanity mode was added. Before, you had to do a custom challenge to get the multiplier high enough. Try making a custom difficulty instead (you can just copy the insanity mode settings if you wish) and use that instead. I have a hunch that will fix the issue. Lots of doing the Apocalypse challenges is re-rolling for workable objectives. Set game mode to insanity, that's the only regular difficulty to do apocalypse. The only objectives I wouldn't recommend are ghost event and escape.
